mmHg Renews SMBP Collaboration with Settlement Health

mmHg is pleased to announce renewal of their agreement with Settlement Health to provide remote patient monitoring services for Settlement’s SMBP program.  

mmHg’s Sphygmo remote patient monitoring (RPM) platform will be used by patients to collect and send health vitals data to providers, who, in turn, will utilize mmHg’s web-based clinician portal to assist with medical decision making.  Identification of patients with white coat hypertension or white coat effect and improving BP control in patients with elevated home BP are the two major use cases.  EMR integration represents an additional collaborative focus.

 

Settlement Health and Medical Services, Inc. is a federally qualified health center dedicated to providing high quality primary care to East Harlem ever since it was incorporated in 1978. They provide a full range of primary care family medicine services including Women’s Health and Prenatal Care, Pediatrics, Adolescent, Adult and Geriatric care.

mmHg is honoured to be working with exceptional partners like Settlement to make SMBP programs feasible and sustainable for underserviced regions of the US.  For mmHg, this brings the total number of FQHC (or similar) partnerships to 20.
